Thinking about getting dermal fillers? They can be the perfect solution to a recessed chin, a sunken cheeks, thin lips, and much, much more. Fillers can do a lot for you—inside and out. If you’ve always been bothered by, say, your lips or your nose or just feel like your face is looking a bit worse for wear, dermal fillers can be just the cure. These injectables lift and add volume to areas of your face to turn those problem areas around. If you’re interested in dermal fillers but aren’t quite ready to commit, here are three things you should know that might get you to hop off the fence.

1. THERE’S VERY LITTLE DOWNTIME

With surgery and some other methods of dealing with aging skin, there’s ample downtime. You have to go through a recovery phase and it’s a pain—in more ways than one. With dermal fillers, that’s not an issue. There’s virtually no downtime at all. You can get dermal fillers and go right back to work. The treatment is also pretty much painless. While dermal fillers don’t last forever, they can last for up to a year or even a year and a half. It all depends on your age, the depth of your wrinkles, the treated area, and what you’re looking for. Now, a year may not seem like a very long time, but think about it, having dermal filler isn’t much different than your yearly visit to the dentist or doctor. Couple this with no recovery time and having an annual filler date isn’t a big deal—especially when you consider the incredible results a good provider can achieve.

3. dermal fillers are completely safe

Many studies have been done on dermal fillers, and they all come to the same conclusion—fillers are completely safe. (Just make sure you always go to a trained, licensed professional!) So safe, in fact, that I’ve written a whole blog about it. You might be wondering about allergic reactions and infections. Fortunately, very, very few reactions have been reported. And because the treatment is non-surgical, infections are extremely rare as well, so no need to be concerned. Side effects are also really uncommon. And even if you do experience any side effects, they last just a few days. You might see some bruising, have a bit of redness, feel some itching, or the area could feel a little tender, but that’s usually it. The signs of aging are slowly creeping up on you. You look in the mirror and see them. Your skin is starting to lose its elasticity. Fine lines are starting to bloom around your eyes and mouth. You raise and lower your eyebrows, and there are lines that take a while to smooth out. The areas under your eyes look dark and hollow, and concealer just isn’t cutting it anymore. But what can you do?

You’ve heard of dermal fillers, injectable treatments meant to plump up your skin by replacing the collagen you have lost over the years and lessening the appearance of fine lines and wrinkles. But what comes to mind when you think of these injections? Some people still think of dermal fillers as torturous treatments where silicone is stuffed into your face, giving you an obviously fake (and often lifeless) appearance. There are also many horror stories about dermal filler injections gone wrong, and patients being permanently disfigured and other terrible outcomes. Needless to say, who would want that?

As a PA-C who has been working with dermal fillers for many, many years, I’m here to set you straight. Dermal fillers are now SAFER THAN EVER (she shouts from the rooftops!) and the difference they can make to problem areas is almost magical.

 

A Brief History of Dermal Fillers

Dermal fillers are the result of years of research and development. But how did they get started? Believe it or not, they got their start back in the late 19th century with fat transplants. In 1893, a German doctor by the name of Franz Neuber used autologous fat (using the same fat from the person receiving the transplant) to fill out facial defects. From there, injections took off.

In the early 1900s, an injectable paraffin was being used. But this was short-lived. Once granulomas (inflamed nodules in the skin) were discovered, that was the end of that. Right around the 1940s was when silicone started to be investigated. It was used all the way through the 1960s, until it started to show adverse effects. The use of silicone was then promptly banned.

Research on the use of animal collagens began. By the 1970s, there was enough research to actually inject them into patients. The use of injectable bovine collagen was developed in the 70s, and became FDA approved in 1981.

In the beginning of the 21st century, research was starting on non-bovine collagen alternatives, including hyaluronic acid. There is now an array of different dermal fillers, each with their own properties.

 

What’s in Dermal Fillers, Anyway?

There is a vast array of different FDA-approved dermal fillers. Here’s a glimpse as to what’s in them.

Many dermal fillers are made with hyaluronic acid, including Juvederm, Restylane and Belotero.  This is a naturally occurring substance that is already found in human skin, which helps to keep it full and hydrated. The filler itself is soft, and almost gel-like in texture. The results provided by HA injections are temporary, lasting approximately 6 to 12 months.

Calcium hydroxyapatite is used in Radiesse. This is another naturally occurring substance, found mostly in the bones. The filler has a thicker consistency than hyaluronic acid, and provides longer lasting results. It also has the ability to stimulate your body’s own collagen production.

Poly-L-lactic acid, found in Sculptra fillers, is a synthetic material, but it is biocompatible. This means that it’s completely safe for use within your body. The filler stimulates your body’s own collagen production and results are long-lasting (up to two years).

Polymethylmethacrylate is another synthetic, biocompatible filler, and is the main ingredient in Bellafill. Fillers with polymethylmethacrylate also contain collagen, and stay in the body indefinitely, providing continued support to alleviate wrinkles and lines.

There are also autologous fat injections, which use the fat within your own body. These injections require a minimally invasive fat retrieval procedure, but the results last a long, long time.

 

The Importance of a Trained Professional

Despite the fact that dermal fillers are completely safe, you still hear horror stories. Why is that?  Well, dermal fillers can’t be injected by just anyone.

You’ve probably heard of dermal fillers, those magical injections of hyaluronic acid (HA) that restore volume to your face, making you look younger by smoothing your wrinkles. I know your mind is screaming, “Run, run, run to get dermal fillers!” Great! But did you know that your facial structure matters a whole lot when it comes to dermal fillers? Let’s step back for a moment and talk about facial structure before you dive in face first. So, how does the structure of your face matter? You may come into Clarity complaining of your jowls, and expect me to start working in that area. Wrong. A great medical aesthetics provider will know to find the “problem behind the problem,” which, in this case, is the loss of volume in the cheeks. Taking stock of your specific facial anatomy and anatomical proportions is an absolute must before any injections begin. Skilled medical aesthetics professionals have started not just looking at wrinkles and problem areas in isolation, but looking at the whole picture, deducing how volume loss in one area of the face affects other areas. (Tip: if your medical aesthetics professional doesn’t do this, run for the hills!)  

A Quick Background on Dermal Fillers

The American Society of Plastic Surgeons states that “Injectable dermal fillers can plump thin lips, enhance shallow contours, soften facial creases, remove wrinkles and improve the appearance of recessed scars.” Here are some of the areas where dermal fillers traditionally work well:
  • lines from nose to mouth
  • lines from mouth to chin
  • lines above upper lip
  • frown lines
  • add volume or definition to the lips
  • restore volume to cheekbones or cheeks
  • under the eyes
  • fill out the back of hands, making veins look less severe
Think dermal fillers are just for your cheeks and lips? You’re wrong! Here’s the scoop on those under-the-radar areas where dermal fillers can work their magic. Dermal fillers are minimally invasive, which means they’re virtually painless and require little or no downtime. While everyone’s skin sensitivity varies, we can’t promise that you won’t experience a slight amount of discomfort. But nothing some distracting conversation, stress balls and—oh, yes—some topical anesthetic can’t handle! And don’t confuse dermal fillers with BOTOX®. That’s just plain wrong. BOTOX is a neurotoxin, which is used to stop muscle movement that causes wrinkles, while fillers plump and smooth sagging skin. Dermal fillers normally last anywhere from 6 months to 5 years. The reason why there is such a time gap here is because there are many different types of fillers that work best for different areas, and individual results vary depending on your aesthetic wants and needs.  

Facial Structure and Dermal Fillers

Now, back to how facial structure matters when it comes to dermal fillers. Normally, when patients come in for dermal fillers, they have lost the “triangle of youth” and now show off a “trapezoid of youth”—meaning that they have lost cheek volume and height, and bone density. To determine what areas of your face need to be augmented with filler, your medical aesthetics professional should use a system called Phi proportions. You’d probably snooze through a scientific explanation of Phi proportions, so to sum it up, Phi proportions allow your medical aesthetics professional to use measurements to figure out what areas of the face are causing problems. These measurements include brow height and length, the length of the nose, peak of the cheek, size and shape of lip, lip and chin length on a lateral profile, and chin length, also on a lateral profile. Your PA-C will then give you a realistic idea of what you should expect to see after the procedure. Everyone’s face is different. And treating them all the same is a surefire way to create cartoon character faces instead of enhancing what’s already there (and beautiful!). Dermal fillers aren’t one-size-fits-all treatments. Natural-looking filler treatments are created by customizing the treatment areas and filler types for you—and only you. There’s a lot of bad dermal filler advice out there. It’s important to do your due diligence before letting just anyone have at it. Does trying to figure out the difference between dermal fillers and neurotoxins leave a permanent furrow in your brow? If you’re considering injectable treatments but don’t know where to start, you’ve come to the right place! Part of my job is to help you understand the difference between the types of injectables we use during cosmetic procedures. The two categories of injectables are neurotoxins and dermal fillers. And while they are both among the most popular anti-aging treatments used today, they are different beasts! Here’s a little “Injectables 101” for your reading pleasure…

How are dermal fillers and neurotoxins the same?

Well, they both:
  • Are minimally invasive with no downtime
  • Not permanent (treatments have to be repeated to maintain effect)
  • Offer natural-looking results
Is that where the similarities end? Short answer: pretty much. Here’s some background to explain more.

What are dermal fillers?

As we start to age, our bodies aren’t able to produce collagen, hyaluronic acid and elastin. Collagen and elastin are essential for tight skin, and hyaluronic acid provides us with plump, smooth skin. Unfortunately, in our 30s, not only does our skin lose the ability to produce these as efficiently as before, but cell turnover will start to slow as well. As a result, we start to see expression lines around the eyes, forehead and mouth. Dermal fillers are made of hyaluronic acid, which is a naturally occurring substance in our bodies. There are also hyaluronic acid fillers that contain the numbing agent lidocaine (to keep you a little more comfortable during your treatment!). According to the American Society of Plastic Surgeons, dermal fillers can be an effective way to:
  • Plump thin lips and restore a youthful contour
  • Restore lost volume to cheeks and undereyes
  • Soften and smooth facial creases and wrinkles
  • Improve the appearance of recessed scars
The FDA has approved the use of dermal fillers to safely and effectively treat wrinkles when injected into certain areas of facial tissue. There are many types of dermal fillers offered, we like and use:
  • Juvederm® Ultra XC

  • Restylane®
  • Restylane® LYFT

What are neurotoxins?

Neurotoxins are also used to treat the signs of aging, except this treatment is administered with a substance called Botulinum Toxin Type A. According to the American Board of Cosmetic Surgery, these injections are the most popular cosmetic treatments administered in the United States. Neurotoxin injectables are most commonly used to smooth wrinkles that are a result of frowning, smiling, squinting or other common facial expressions. When this treatment is administered, the neurotoxin blocks nerve impulses to the facial muscles, which causes the muscles to relax. So, in turn, expressions soften and wrinkles are drastically reduced. It’s important to remember that the effects of neurotoxin injectable treatments aren’t immediate. Full results will appear within one to two weeks. The neurotoxin treatments that we like and use include:
  • BOTOX®
  • Dysport®
  • Xeomin®
Although these injectable treatments are commonly used to treat wrinkles, they can also be used to treat excessive sweating, TMJ and soften vertical neck bands.

What are the key differences between dermal fillers and neurotoxins?

Both dermal fillers and neurotoxins are used to treat the effects of aging but in different ways. Here are a few key differences between the two injectable treatments. The difference in the time it takes for the treatments to take effect. When a patient receives neurotoxin treatments, it will usually take a few days to take effect. In most cases, the full effects of neurotoxin injectables can be seen two weeks after treatment is administered. One the other hand, the results of injectable fillers can start to appear instantaneously. However, when fillers are administered, it’s typical for swelling to occur within the first few days. Neurotoxin injectables are usually a better option for those who want to reduce the appearance of dynamic wrinkles. This is because it will reduce the motion of the facial muscles. The forehead, between eyebrows or “11’s” (as this area is commonly called) and crows feet are ideal areas to treat with neurotoxin injectable treatments. Dermal fillers are usually better suited for static wrinkles. These are wrinkles that are visible even when there’s no expression and the face is relaxed. These areas will benefit the most from dermal fillers because the fillers will bring fullness to areas of the skin with static wrinkles. Dermal fillers and neurotoxins are both ideal ways to treat wrinkles and recapture a youthful glow, but only an experienced medical aesthetics provider knows how to wield a needle to achieve the best possible results for you. Combination Therapy – What is it?

Combination therapy quite simply involves using two or more anti-aging treatments to help treat the various signs of aging. Why is Combination Therapy the Best Treatment?

Overall facial aging is the result of several different factors. First, loss of volume, supportive collagen, fat and bone loss occurs and will cause hollows and sagging skin.  Second, years of muscle movement will cause wrinkles such as forehead lines, crows feet and the lines between your eyebrows or “11’s”.
