Dermal fillers are one of the most popular aesthetic treatments available. They are used to treat a variety of different trouble spots, including lines, wrinkles, undereye hollows, sunken cheeks, and more. With dermal fillers, you can achieve smoother, younger, plumper looking skin. However, social media has resulted in an increase of pressure for people to get fillers and to get over filled. It’s becoming increasingly more common to see patients with too much filler, an issue that can result in what is known as “pillow face.” More is not better! Here’s what you need to know about pillow face syndrome and what you can do to prevent it.

What is Pillow Face Syndrome?

“Pillow face syndrome” (as referenced by Dr. Shinzo Bay Aguilera, a famous dermatologist in Fort Lauderdale) occurs when too much filler has been injected. The result is a facial appearance that resembles a “fat feline.” This is a widened face with narrowed eyes, overfilled lips, and a pointy chin. It’s a very unnatural facial shape that has become the norm through social media. In fact, many injectors tend to overfill simply because they think it’s the look their patients want. This is why choosing the right injector is so crucial. The best injectors will assess your facial features to best determine your needs. They’ll ask about your goals and discuss realistic results rather than just assume what you want. Don’t be afraid to ask any injector questions about their experience or to see before and after pictures during your consultation.

More is NOT better!

The best fillers provide subtle, natural results. Too much filler changes the shape of your face. Once injected, fillers don’t blend together, they actualLY layer on top of each other. Each time it’s injected, it adds to the previous product placed. Why is this important to know? Sometimes we see patients who bounce around from injector to injector. Not all injectors are skilled at spotting a “filled face”, and place more filler on top of filler, this can lead to your facial shape being altered. Find an injector whose work you admire and trust. Allow this injector the time to work on your goals. Most natural fillers last around 6 months before they begin to break down and absorb into the body. Synthetic fillers have been made to last longer, some lasting as long as 18 months to 2 years. It’s best to wait an appropriate amount of time between injections to help keep you looking like yourself. In the event of an overfill, some fillers can be reversed. Dermal fillers made using hyaluronic acid (such as Juvederm® and Restylane®) can be broken down with another injection called hyaluronidase. Keep in mind that this antidote eliminates all filler, so you will need to start over from square one. For synthetic dermal fillers (including Radiesse and Sculptra), there are no antidotes. Instead, you will have to wait until the results wear off on their own.

Lumps and Bumps Following Dermal Fillers

Lumps and bumps are a sign that you’ve gone too far! Some mild swelling and bruising are common after filler injections. You may even notice some slight unevenness. A good injector can help to minimize these side effects. Should soft lumps develop, they can generally be massaged to help disperse the filler more evenly. However, too much filler can result in more significant changes. If your skin starts to feel firm, hard, and unnatural, this is a sign that too much filler (or now subsequent scar tissue) has replaced or displaced your own tissue.

Let’s be honest here for a second. When you hear the words “aesthetic treatments,” what’s the first thing that comes to mind? Typically, most people equate aesthetic treatments with women desiring a more youthful appearance. While we do see a lot of women, men are undergoing treatments too. In 2017, over 1.1 million men had minimally invasive procedures performed and 214,434 surgical procedures done. While men only make up 13% of the total number of procedures performed, these numbers are three times higher than they were in 1997. The taboo is disappearing. Men, especially over the age of 50, are starting to take steps to stop the aging process. Their reasons aren’t to look super young though. Their goal is generally to look more relaxed and rested. With today’s advances in non-surgical aesthetic treatments, we have the tools to deliver subtle, natural enhancements that preserve a masculine look.

1. Botox®/Dysport®/Xeomin®/Jeuveau™

These are all injectable treatments that are also known as wrinkle modulators. They involve the use of a purified protein that works to temporarily suspend muscle movements at the locations where they are injected. Strategically placed, these toxins are able to soften crow’s feet, relax forehead lines, and ease 11’s (the lines that appear between your eyebrows). Treatments are quick, typically only taking about 15 minutes to complete. They are virtually painless and there’s no downtime. It takes about 2 to 10 days for wrinkle modulators to take effect and results last anywhere from 3 to 6 months.

2. Hair Treatments

Some men have unwanted hair. Others are losing hair and want to get it back. We offer solutions to meet both of these needs. Laser hair removal is done to eliminate hair where you don’t want it (such as your ears, back, or unibrow). Instead of waxing, shaving, or plucking, our laser offers permanent hair reduction. Laser light passes through the skin and is absorbed into the hair follicle where it is turned into heat. The heat disables the follicle, preventing new hair growth.

For those looking to regrow hair, we offer PRP with microneedling to awaken sluggish hair follicles and encourage hair regrowth. PRP is a procedure that involves using a small sample of your own blood. Your blood contains platelets, which contain growth factors that aid in tissue healing and regeneration. 4. Dermal Fillers

Dermal fillers are injectable treatments that are placed to fill areas of volume loss. The answer is each dermal filler has a unique consistency and molecule size. This is important because each problem area such as undereye hollows, a weak chin, and smile lines require  filler to be placed at different depths under the skin. Lip injections have become an increasingly popular treatment. They provide patients with a treatment option to augment what Mother Nature gave whether that be thin lips, asymetrical lips or loss of detail (typical as we age). If you’re self-conscious about your smile, you may have considered the treatment yourself. While lip injections involve the use of fillers, which are minimally invasive, this treatment is no laughing matter. Choosing an experience provider is crucial. The treatment is a process, and skipping a step along the way can lead to less than perfect results. At Clarity, we want you to understand what’s involved during the process of lip injections and what to expect every step of the way so that you get the best results possible.

Find an Expert

Before you do anything, you need to find an expert injector, like Kati. Conduct a search online and review the websites of several providers. Many providers will post before and after photos on their website so that you can see just what types of results they can produce. This will give you a good idea of the results the provider is able to achieve and if their aesthetic matches your own. It’s important that you like what you see before you start making any phone calls.

Schedule a Consultation

Your next step is to schedule a consultation. During your consultation, you will be given an opportunity to discuss the goals you have for your lips. An honest injector will tell you whether or not your goals are in harmony with your facial anatomy. For example, very thin lips on a small face are best suited for a subtle enhancement performed over time. Expert injectors have a keen eye for assessing your facial features and what types of fillers will provide you with the best results. If you are happy with your consultation and decide to move forward, your “before” photos are taken.

At the Time of Your Treatment

When you come in for your treatment, you will be given the option of a topical anesthetic OR  an injection of nerve-blocking agent (such as lidocaine)  both meant to keep you comfortable throughout your procedure. Your filler is injected using a very thin needle according to your customized treatment plan. Ice is provided immediately following your injections. This is done to help minimize the amount of swelling and bruising you experience.

After Your Treatment

After your treatment, YOUR LIPS WILL BE SWOLLEN. Unfortunately, there is no way around this fact. And, bruising is also possible. We are sticking needles in your lips, one of the most vascular areas on your body. It’s actually very common to hate your lips until the swelling goes down. Swelling can be disproportionate and is usually at its worst the following morning. It can take 2 to 3 days for your swelling to go down, although it’s not uncommon for some swelling to linger for about 2 weeks. Ice packs can help. However, it’s important to avoid applying significant pressure to your lips. You should keep your lips clean and avoid excessive touching and being overly expressive with your mouth, we want your filler to seat properly.

Schedule Your Follow Up

The last step of your lip injection treatment is to schedule a follow-up. Your follow-up is the time for you to assess what you liked, if you want a bit more enhancement, or if there is an asymmetry that you want to be addressed. We will also take your “after” photos. You’ll be able to see these photos right next to the “before” photos that were taken during your initial consultation. Seeing these pictures side by side will allow you to really see the difference. It’s amazing just how quickly you forget what you looked like before your treatment!

Lip injections can help to completely transform your lips, but it is a process. It’s important that you don’t skip any steps. Doing so could lead to a less desirable result than you expected. You also don’t want lip injections done by just anyone. As we mentioned earlier, the lips are very vascular, a trained injector like Kati can recognize when something doesn’t look quite right and take steps to rectify the situation immediately. Getting older is something we can’t really control. While some really great things come with getting older, like wisdom, more freedom, and grandchildren, there are some not so great things that happen too. As we age, so does our skin. Over time, collagen production slows. When this happens, our skin loses its ability to bounce back and begins to sag. Eventually, you may begin to notice the formation of a double chin.

FaceTite by InMode is a facial contouring treatment. It uses radio frequency assisted lipolysis, or RFAL for short, to destroy fat cells and tighten loose, sagging skin. The treatment involves the use of a two prong applicator to deliver radio frequency energy. This applicator consists of a long, thin tube, or cannula, that’s inserted under the skin and an electrode that passes over the skin’s surface. The skin is essentially enveloped in radio frequency energy. The radio frequency energy melts fat cells and stimulates collagen production. The procedure is quick and minimally invasive. AccuTite

AccuTite is from the makers of FaceTite. It’s a smaller version of FaceTite, the “little sister” if you will, and is used to treat smaller areas. While a smaller tool, it is still incredibly mighty. Just like FaceTite, AccuTite uses RFAL to melt fat cells and tighten skin. After a local anesthetic, a small, thin tube goes under the skin and an electrode goes on the surface. Your skin is wrapped in radio frequency energy, liquifying fat cells and stimulating collagen production. Just like FaceTite, results are immediate. The best results are noticeable after 6 weeks and continue for up to 6 months. AccuTite is excellent for mild to moderate skin laxity and/or fatty deposits. So, if your double chin is just starting to form, this may be the right treatment for you.

CoolSculpting®

CoolSculpting® is a completely non-invasive way to get rid of stubborn, unwanted fat that won’t respond to diet and exercise changes. There’s a common misconception that CoolSculpting® is only effective on the belly, but it’s actually a perfect treatment for a double chin, one of the most difficult areas to treat no matter how many dietary changes you make or how much you exercise.

This incredible treatment works by freezing fat away. During the treatment, a vacuum applicator is applied to the skin that allows it to be exposed to the cooling panels that are used to freeze the fat. There’s no need for any needles with CoolSculpting®. Once the fat cells freeze, they die. Your body then flushes them out naturally. The most drastic results occur after the first two to three months after your treatment. Many patients continue to see additional improvements in their appearance and profile another one to two months later.

Kybella®

Kybella® is specially designed to eliminate fat cells in the submental area (the dreaded double chin). It’s the only FDA-approved treatment to do this as well. But, how? Kybella® is an injectable treatment with synthetic deoxycholic acid as an active ingredient. This ingredient works to destroy fat cells, effectively melting away your double chin. As the dead cells are then flushed out of the body, leaving you with a much more sculpted jawline and greatly improved profile. Depending upon the severity of your double chin, a second treatment may be recommended four weeks later. The results are typically noticeable four to six weeks after your treatment and your double chin continues to decrease oer the next 6 months.

Morpheus

Morpheus uses fractionated radio frequency to direct radio frequency energy deep into your skin, right to the fat layer. This is an ablative treatment, meaning that the skin is perforated in a controlled manner to accelerate cell turnover on the skin’s surface. The gold-plated steel micro-pins on the head of the handheld device also deliver radio frequency energy to preselected depths at the same time. The radio frequency energy stimulates collagen and elastin remodeling. Morpheus is one of the most advanced ablative radio frequency treatments available. It can be very effective for reducing your double chin, smoothing and contouring the area for a much-improved appearance. In general, one treatment is recommended per decade of life. Treatments are spaced out every four to six weeks. You may begin to see your double chin disappearing as soon as two weeks after your first treatment. The most noticeable difference is after your last treatment. Your double chin will continue to improve for up to three months after your treatment cycle is over.

2. PDO Thread Lift

A PDO thread lift involves the use of polydioxanone (PDO) suture threads to lift and reposition the skin on the face, chin, and neck. The PDO threads are inserted under your skin in a lattice pattern. They then act as a scaffold that holds your skin up against the natural effects of gravity. After the threads are inserted, they stimulate your body’s natural collagen production. The threads are absorbable, and gradually absorb within four to six months, leaving behind the newly formed scaffold, which continues to help keep your skin lifted for up to the next 12-24 months. AccuTite is the little sister to FaceTite. Just like FaceTite, it utilizes RFAL technology to tighten skin and liquefy fat cells under the skin. And like FaceTite, AccuTite uses a double-sided probe to deliver radio frequency energy both internally and externally. However, AccuTite is specifically designed to treat smaller areas of the body. It is a small, but mighty, tool that can lift your brows, tighten droopy lids, and reduce bags under the eyes, replacing surgical skin tightening procedures like a blepharoplasty (eye surgery) or browlift.

4. Liquid Rhinoplasty

Whether you’ve always been unhappy with your nose or it’s changed due to a traumatic injury or getting older, this unhappiness can have a substantial impact. Your nose is incredibly prominent, and it’s pretty difficult to hide. For a long time, the only solution to change the nose was a rhinoplasty, or surgical nose job. A surgical nose job is a permanent change to your face, and this isn’t always a good thing. Enter non-surgical rhinoplasty. Also called a liquid nose job, this treatment uses dermal fillers to alter and improve the shape of your nose. Dermal fillers can help to eliminate the appearance of bumps or contour and reshape the nose. They can also help a slightly crooked nose appear straighter or soften sharp angles. The results are magical. Please note that a liquid nose job cannot reduce the size of a large nose, so if a smaller nose is what you’re after, your best bet may still be surgery.

Who is a Good Candidate?

Candidates for a non-surgical nose job are those whose nose has a bump, is mildly crooked, has a droopy tip, or has sharp angles. Dermal fillers can be used to fill in the areas around the bump, eliminating its appearance. A filler can contour and reshape the nose, helping your nose look straighter and more symmetrical. A filler can also provide lift or smooth out the sharp angles. It’s kind of crazy what a little filler can do!

Who is NOT a Good Candidate?

While a small amount of carefully placed dermal filler can make a huge difference for many patients, a non-surgical nose job isn’t necessarily the right solution for everyone. A liquid rhinoplasty can’t make a large nose smaller or make a wide nose narrower. Patients with these issues should consult with a plastic surgeon. In these types of cases, a surgical rhinoplasty is probably the best answer. Still, we offer free consultations before all injectable treatments. During the consultation, we review your challenges, worries, and hopes to make an educated decision about whether a non-surgical nose job is right for you.

Is a Non-Surgical Nose Job Safe?

Dermal fillers are now safer than ever, and the difference they can make is almost magical. Long gone are the days of silicone being injected into the skin, creating an obvious fake and lifeless appearance. We even have alternatives to the bovine collagen injections that were FDA approved in 1981. Today’s dermal fillers use naturally occurring or biocompatible substances. Many are made with hyaluronic acid, a substance that naturally occurs within the human body to plump and hydrate the skin.

Dermal Fillers: What’s the Difference?

The Types of Dermal Fillers

  • Hyaluronic acid – Hyaluronic acid, or HA, is a naturally occurring substance that’s found in both humans and animals. This substance binds to water and gives skin its plumpness.
  • Calcium hydroxyapatite – This is a mineral-like substance that’s naturally found in your bones. As a filler, calcium particles are practically microscopic and suspended in a smooth gel. CaHA fillers are thicker than HA fillers and are used to treat deeper lines and wrinkles. These types of fillers also typically last longer as well.
  • Poly-L-Lactic acid (PLLA) – PLLA is a synthetic biodegradable substance that’s also biocompatible (it’s completely safe to use in your body). This substance has actually been used in medical devices for years, such as dissolvable stitches. PLLA fillers are considered to be collagen stimulators, meaning they help your body to rebuild its own natural collagen.
  • Polymethylmethacrylate (PMMA) – PMMA is another synthetic biodegradable substance. In fillers, PMMA takes the form of tiny spheres that remain underneath your skin to provide support. These fillers also contain collagen to add structure and a firmer appearance.

Small Molecule vs. Large Molecule

Different dermal fillers come in different molecular sizes. These differences in size mean that dermal fillers, while achieving a similar goal, behave a bit differently from one another. Larger molecule dermal fillers are thicker. This makes them better suited for deeper lines and wrinkles, and for targeting more specific areas. Fillers with smaller molecule sizes are thinner. This means they spread more easily and can treat a larger area.

Superficial Fillers vs. Foundational Fillers

Superficial fillers are those meant to treat finer lines and wrinkles in your skin. If you have shallow lines and wrinkles, these types of fillers are perfect for giving you just the right amount of lift. Foundational fillers, on the other hand, treat lines, wrinkles, and sagging at a foundational level. These fillers are thicker, and help your body to produce its own natural collagen. They also typically last longer than superficial fillers.

Short-Term Fillers vs. Long Term-Fillers

Short-term fillers are those that are absorbed by the body over time. They last varying lengths of time. Depending upon the dermal filler, they can last anywhere from 3 to 18 months. Long-term fillers are made from synthetic materials don’t get absorbed by your body. They can actually remain in your body for several years.

Short-Term Fillers

Belotero Balance®

Belotero Balance is a hyaluronic acid filler. It’s a small molecule, superficial filler that can spread more easily in the skin. It is often recommended to treat pesky fine lines in your skin. Results are immediate and can last as long as 9 months.

Juvederm® Ultra XC

Juvederm Ultra XC is made with hyaluronic acid and has a small molecule size. It’s a superficial filler. It can be used to smooth lines, ease nasolabial folds, add volume to cheeks, and plump your lips. Results can anywhere from 6 months to a year.

Juvederm® Volbella

Juvederm Volbella is a hyaluronic acid filler that has a small molecule size. The concentration of HA is also lower in this particular. It’s a superficial type that is specifically designed to treat vertical lip lines and restore a natural, subtle shape (and fullness) to your lips. The results of this filler last about a year.

Juvederm® Vollure XC

Juvederm Vollure XC, like other Juvderm fillers, is a hyaluronic acid filler. This particular filler contains a mixture of small and larger molecules, and is thicker than Volbella. It’s still considered a type of superficial filler. With Vollure, we can treat moderate to severe facial wrinkles and folds. Specifically, we can use Vollure to treat smile lines, marionette lines, and nasolabial folds. Vollure lasts anywhere from 6 to 18 months.

Radiesse®

Radiesse is a calcium hydroxyapatite filler. It has larger molecules and is a foundational filler. It adds volume while helping your body to stimulate its own natural collagen production. Radiesse works best for treating deep wrinkles and folds. It can be used to restore volume all over the face as well as volume in your hands. It can even be used to treat the ears and fill in scars. Results last 7 months to a year.

Restylane®

Restylane is a hyaluronic acid filler with a larger molecule size. It’s a superficial filler that can treat moderate to severe lines and wrinkles. This includes nasolabial folds and marionette lines. It can also be used to augment lips and treat dark circles under the eyes. The results of this dermal filler typically last 6 to 9 months.

Restylane® DEFYNE/REFYNE

Both Restylane DEFYNE and Restylane REFYNE are hyaluronic acid fillers. DEFYNE has a larger molecule size, while REFYNE contains smaller molecules. DEFYNE is better suited for achieving lift and volume where REFYNE can help to treat marionette lines, nasolabial folds, and tear troughs. These fillers use XpresHAn technology, which gives just the right amount of lift while still allowing you your natural range of facial movements. Results last up to a year.

Restylane® LYFT

Restylane LYFT is a hyaluronic acid filler. It’s a superficial filler with a larger particle size. We use LYFT to provide fullness to your cheeks and smooth out smile lines. It essentially provides a cheek augmentation. The subtle, natural results last up to a year.

Restylane® Silk

Like all other Restylane fillers, Restylane Silk is a hyaluronic acid filler. The molecules in this superficial filler are smaller. While not a big volumizer, Silk is an incredibly effective treatment for treating issues with your lips, such as thinning and lost shape. It can also help to define the cupid’s bow and smooth out lines around your mouth. Results last about 6 months.

Long-Term Fillers

Artefill®/Bellafill®

Artefill and Bellafill are Polymethylmethacrylate fillers that consist of PMMA microspheres that are suspended in bovine collagen. The molecules in these fillers are larger in size. They’re also foundational fillers, which provide an internal support structure that prompts your body’s natural collagen production while plumping your skin. Artefill and Bellafill can restore volume as well as plump deep wrinkles and scars. They can also be used to augment the nose, chin, and jaw line. Results from these fillers can last up to 5 years.

Juvederm® Voluma XC

Juvederm Voluma XC is a hyaluronic acid filler. It has a large molecular size, and is the thickest of the Juvederm line. It’s a superficial filler that’s designed to treat signs of aging in the cheeks and mid-face region. It’s best suited for lifting and contouring, and does wonders for the most noticeable wrinkles and folds. It can even be used to help augment your chin. Results of Voluma can last up to 2 years.

Sculptra® Aesthetic

Sculptra Aesthetic is a filler that uses Poly-L-Lactic acid. It’s a small molecule filler that is also a foundational filler. It’s referred to as a facelift in a bottle, or a liquid facelift. As a PLLA filler, Sculptra Aesthetic works to stimulate your body’s own natural collagen production, restoring the skin’s internal structure and improving volume gradually over time, over the course of 3 treatments. This particular filler is a global filler, meaning that it can be used to treat many different areas of the face. It’s not very effective for spot-treating specific areas, but it can be incredibly effective for treating larger areas. It can treat volume loss, facial asymmetry, collagen loss, temporal hollows, and sharp features. Results can last up to 2.5 years. Your cosmetic needs and goals are unique to you.
